Alamo Heights Health and Rehabilitation Center is a 162 bed skilled nursing facility, licensed by the State of Texas, providing both long-term and short-term care. We are dedicated to optimizing the health and quality of life of each resident through individualized care planning and treatment in a safe and therapeutic environment. Our programs are directed by independent, board-certified physician specialists with the guidance of an independent Medical Director.
Our staff members are trained to care for individuals with varying needs, from residents who need basic assistance with their activities of daily living to those who require more complex medical care. We know that treating the whole person involves more than medical treatments.
A team of therapists, under the direction of an independent physiatrist and the support of certified therapy assistants, develop a plan of care for each patient. We offer Physical, Occupational and Speech therapies. Therapy is offered up to three (3) hours a day and up to six (6) days a week depending on the resident's need and as ordered by their physician.
The facility's Pulmonary Rehab Program manages the unique needs of residents with tracheotomies. Under the direction of an independent pulmonologist, a respiratory therapist works with specially trained, licensed nurses to continually monitor this resident population. Our services include, but are not limited to, accommodating residents with tracheotomies, t-pieces, bi-pap and c-pap therapy.
Under the direction of an independent wound care physician, our licensed wound care nurses work seven (7) days a week for residents requiring treatment for complex wounds, including but not limited to post-operative wound sites, diabetic and decubitus ulcers.
Long-term care assists those who may benefit from 24-hour nursing availability and assistance with meals, medication management, and other activities of daily living.
The environment at Alamo Heights Health and Rehabilitation Center is designed so residents may live comfortably as part of a community.
Alamo Heights Health and Rehabilitation Center provides various recreational activities to meet each resident's spiritual, social, sensory, cognitive and physical needs. Activities are adjusted to accommodate each resident's level of ability. We provide individualized, and group activities.
Our recreational program includes music, arts, crafts, games, religious services and outings. Residents receive a monthly calendar of events and reminders so they can participate in events. Activities help bring residents together and greatly improve their quality of life.
